The Diabetes Self-Management program at Hilton Head Hospital is an outpatient program designed to teach people how to successfully manage their diabetes.
Our program provides training on proper nutrition, blood glucose monitoring, physical activity, stress management and preventing diabetic complications. Motivation and support are provided to allow for positive lifestyle changes.
Professional staff includes a certified diabetes educator, registered nurses, registered dietitians and exercise physiologists. Together this professional team provides a well rounded approach to helping you control diabetes.
The Diabetes Self-Management Program is recognized by the
American Diabetes Association and maintains its national standards.

Understanding Diabetes
Balanced Nutrition Meal Planning & Carbohydrate Counting
Living Healthy with Diabetes
Stress Management
Physical Activity
Insulin Instruction (when needed)
Monitoring and Controlling Blood Glucose
Preventing/Managing Short & Long-term Complications
Local & National Diabetes Resources
